In this final issue of Volume 6 of Grand Rounds in Urology we are pleased to provide our readers with overviews into the treatment of two difficult to treat genitourinary issues: Peyronie’s Disease (PyD) and superficial bladder cancer. In the first article, Drs. Anthony Bella and Tom lue provide a detailed approach to surgical approaches to PyD, a physically and psychologically devastating condition for both the affected patients and their partners. More than a typical review arti- cle, this contribution offers both a review of the current literature and first-hand ex- perience from techniques developed by the authors while at the university of Califor- nia at San Francisco. It is accompanied by several detailed and valuable illustrations.
In the second article, Dr. Ihor Sawczuk and colleagues from the Hackensack (nJ) university Medical Center give a timely update in the diagnosis and treatment of superficial bladder cancer, the fourth most common cancer in men and 12th most common cancer in women. As the recur- rence rates climb to 50% to 70%, and as progression rates rise 10% to 35%, the au- thors provide several practical approaches to diagnosis and treatment in advance of the upcoming aua guidelines, which are due to be published in early 2008. From discussion of new advances and concepts, such as the use of fluorescent cytoscopy, intravesical postoperative therapy, bacillus Calmette-Guerin (BCG) maintenance, re- peat transurethral resection (TuR), early cystectomy, and risk stratification, readers should find this article a valuable overview of topics of current interest. In addition to the regular articles in this issue we have included is a special supple- mental article on updates in the treatment of metastatic prostate cancer, provided by nick Vogelzang and colleagues from the nevada Cancer Institute. as readers will recall, earlier this year in the March issue (Volume 6, issue 1) of Grand Rounds in Urology, we published two articles that dealt with the promising research being conducted in the treatment of hormone- refractory prostate cancer (HRPC). In the first article nancy Dawson presented the “current state” of prostate cancer research and offered readers a thorough overview of the ongoing of newer treatment options. In the second article Dan Petrylak specifi- cally focused on two particular treatment options, Dn-101 and satraplatin. at the time both drug candidates provided great promise for both patients and clinicians. However, as we have so frequently seen in fighting this dreadful disease, early prom- ise can often lead to disappointing results in later trials. Heading into 2008, it seems that neither satraplatin nor Dn-101 will provide clinicians with the additional first-line treatment options we hoped for. However, as Vogelzang and colleagues write, time will tell, and for many other drug candidates discussed in the supple- ment we remain optimistic. Finally, as this is the final issue of 2007 I would like to thank the members of the Editorial Board for their assistance in the development of content, review of sub- missions, and in some cases their direct contributions.
