This issue of Grand Rounds in Urology (GRU) continues our tradition of providing the practicing urologist with the latest in hands-on information. In our last issue Dr. Jeffrey P. Weiss and associates from Cornell presented the first of a two-part series on treatment options for Nocturia. This treatise is concluded in the current issue of GRU. Nocturia is such a common symptom, and Dr. Weiss and his group compartmentalize the etiology of nocturia into 5 major categories. Treatments are discussed, and practical patient examples are explored. Since reviewing their manuscript months ago, I have quizzed some of my colleagues and residents about the classifications of nocturia, and few even came close to being able to categorize the symptom complex. When I was able to enumerate the etiologies, they were impressed. I am sure they wondered how my interests had expanded beyond prostate cancer and BPH. I really believe this two-part presentation is a urologic classic and should be required reading for all of our residents.
The management of renal cell carcinoma has been surgical ever since I entered the field of Urology. There was little hope for control or cure for advanced disease. The Southwest Oncology Group (SWOG) completed a large randomized clinical trial headed by Dr. Robert Flannigan, which demonstrated that nephrectomy in metastatic disease improved survival by 30%. Immunotherapeutic drugs have resulted in some rare responses, but for most no benefit. Thanks to research into the molecular mechanism of renal cell carcinoma, a number of potential targets were identified. Targeted agents were developed, and clinical trials completed. This past year, two new agents were approved for renal cell carcinoma, and others are in various phases of clinical trials. Dr. Lara and associates provide a very comprehensive and yet concise review of all aspects of renal cell carcinoma and share with us the excitement of these new discoveries, which usher in a new era in the management of this cancer.
